By completing and submitting the form below, I request to be considered for the A.L.I.V.E. Coach Trainer Training program. I understand the program is conducted in 12 sessions over the course of 14 weeks and all I need to access the interactive sessions is a telephone, computer, and internet connection. I understand that I will learn, not only the 8-week protocol, but the cutting-edge science that is intended to allow me to further my career. I understand that the entire program valued at $6,000, is available to me, if I am accepted into the group, at $640 (four monthly installments of $160 each). By completing this form I am under no further obligation. I will be billed $12 to attend the first session, Tuesday, July 26, 1:00 PM Eastern Time. If I have interest in continuing in the program, I will email [email protected] expressing my interest and we'll schedule a phone call to determine if this is an ideal fit for me right now. If I am accepted into the program, and I opt to pursue it, the $12 (non-refundable) will be credited toward my first payment.
